The Region 2 DVD has the original European theatrical ending to Eye of the Needle. The ending of the US theatrical version, which is on the R1 DVD, was cut.

In the R1 version, after Kate Nelligan shoots and kills Donald Sutherland, the film ends with an overhead shot of the sea cliff and fades to the end titles, causing a truncation and clumsy edit of Rozsa's music. In the R2 version, after Kate Nelligan shoots Donald Sutherland, a helicopter lands on the beach. Ian Bannen steps out of the helicopter and exchanges a few lines of dialogue with Kate. Distressed, she says "I had to kill him". A cut is then made to her son who is seen clambering down to the beach and she runs, arms outstretched towards him. The scene then cuts to the overhead shot of the cliff followed by the end titles. Rozsa's music continues without interruption throughout these scenes.

The UK London cinema premiere (which I attended) had other alterations to the ending which have never been duplicated on DVD. That print omitted a point of view shot of Donald in the boat and had some addition dialogue as Kate called out to her son. All of which indicates that the studio was not too sure how best to edit the final scenes.
